Job Purpose:
Plan, organize, develop & direct overall operations of the facility's social service department in accordance with current federal, state and local guidelines and regulations.

    Responsibilities:
Advocate for the residents, ensuring residents rights are honored.
Participate in MDS assessments and interviews and the development of individualized plans for residents.
Establish open lines of communication with interdisciplinary team members in order to provide a comprehensive and holistic patient care plan.
Facilitate and coordinate admissions and discharges.
Participate in marketing plan.
Participate in facility's Quality Assurance and Improvement Program.

    Qualification/Education Requirements:
Degree in social services or licensed nurse.
Experience in Long-Term Care preferred
